REQUIREMENTS

  • Must be a resident of the 48504 or 48505 Zip-Codes

  • Children must be under 10 years old

BRING WITH YOU:

  • Proof of Address (drivers license, picture id, consumers or water bill)

  • Your child’s Birth Certificate, Medical Card, or Social Security Card
